This November 9th as we participate in the Welcome Home 5K, a portion of the course has been secured to Honor 250 of our fallen service members with a powerful and moving memorial of 18x24 Honor Signs.
Thanks to the generous support of our sponsor, WILLIAMS, the Honor Signs that will line the final mile of the course will be available free of charge on a first come, first serve basis to anyone who wishes to honor a family member or friend. The Honor Signs will be donated to the the families after the race at the Eagle OPS Rally Point Tent.
Please fill out all the information in this form to submit a request to have your loved one honored during the Honor Mile. Remember - We only have 250 slots available and it is a first come / first serve basis. We will notify you via email if your honor sign will be added to the event. All entries MUST be submitted by October 1st .
When: November 9th - 10am
Where: Mohawk Park, Tulsa
*KIA is NOT a requirement for submission.
*All service eras accepted.
*Honoree does NOT have to be from Oklahoma
*YOU do NOT have to attend the Welcome Home 5K in order to submit a loved one
(although it is encouraged as this will be the only way to see the Honor Mile.)
**Please make sure that the photo you submit is the best quality you are able to obtain. Remember that these will be expanded to a larger size and if the quality is low, the photo will not turn out well. If you need any help with photo submissions, please reach out to to me at [email protected].

Greetings! This will be my fourth year having the honor of taking the lead on Operation: Honor Mile. It has been and continues to be my most significant and rewarding work. It is truly a passion of mine to not only work in our veteran community, but to help ensure that those we have lost are not forgotten and that their families know that they are forever a part of OUR family.
I have experienced thru this process, the many layers of grief that exists, but one thing is always a constant, you want your loved one to be remembered. It has been a blessing to learn so many things about your heroes. Not only their faces and names or details of their service, but a glimpse of who they were, things they enjoyed etc. Thank you for that. There is a place at the bottom of the form for additional information. You are welcome to share anything you'd like there - from service information to a best memory or nothing at all. I just wanted you to know that I have read them all in the past, remember it when I see their faces and truly feel honored to know some things about the men and women who we honor at the Welcome Home 5K. They are not forgotten.
